I use excel on my phone... Works well. Pen and paper easy but I'm more likely to use my phone as its always near.
 
I use Notepad to keep it super sophisticated. I update the weights on there and use my log on this site if I ever want to go back and see the weights I used to use, which is rarely.

I then copy over my current plan to my phone's notepad app for quick reference.

yes, for over two years I have recorded everything n excel - training, sets, reps and weight lifted, intensity (color coded); diet, including macros; body weight in morning.
 
I keep a log religiously.
Pen and paper only.
Tried phone apps but can't find a suitable one.
Google docs, sheets

I still log everything, great to look back at similar milestones and gauge progress/ reassure ability
