Methyl salicylate/diclofenac/other topical NSAIDs will, and wont give you the gastric ulceration issues that oral ibuprofen long-term will. And the point of menthol creams is to overload the nerve carrying the pain signals with burning instead, so you don't feel the pain. It does have a physiological basis.
Half and half. The NSAIDs have a greater effect on the area that you take them, so when you take it orally a lot of the effect goes into the stomach. Technically it could still happen with IV NSAIDs, but those are pretty rarely used and never in the long term.
I've got no idea about the different menthol creams, I've only used a deep heat menthol/salicylate one which was on special at Coles.
Not really. Methyl salicylate+aspirin could possibly push you into acute salicylism/acidosis.
I'd mostly be worried about the gastric ulceration/increased risk of cardiac events from the orals.
