After sending 2 emails I eventually got a response - this code seems to be the problem. What is this code and where does one get it? Email response below:

Can you please explain:
1. What this number is and why it is necessary? In the hundreds of international purchases I have made over the years this has never been raised before.
2. Where this number is obtained and by whom (does the bank provide it to you or do I need to get it somehow)?
3. How many weeks would my order have sat there 'waiting for approval' before someone contacted me if I had not chased it up?

Re: Order as per below
The order below was place on the 12/9/14 and the status still reads as 'waiting for approval'. I queried this through your on-line contact form on the 15/9/14 but have as yet had no reply. Can you please tell me what 'waiting for approval' refers to and the anticipated time to ship, as it has already been nearly a week.
