how do i increase the weight I'm lifting?

Do you usually add weight onto your last set of the exercise?
Or do you
Add the amount on to each set so you end up lifting more in total?

Or any other suggestions welcome...
 
I aim for a specific amount of reps. Let's say 12. Then i know how many sets I'm going to do. Lets say 4. If i can do 4 sets of 12 reps, then I move up the weight. 5 kgs on each side for barbell, and 5 pounds each side for dumbells, and between 5 and 10 ponds/kilo's depending on the machine.
