What's the Basics of a Good Training Program?

In your opinion, what's the basics of a good training program that achieves the following goals:

1) Enhances physical appearance both muscularly and aesthetically

2) Increases strength and other significant markers of performance (speed, agility, flexibility, etc)

3) Improves overall health (all aspects) and body quality (tissue and joint / ligament health, movement fluidity, etc)
 
^ Yep, its the backbone, apply it to intensity, frequency, good diet and results are likely to follow.
 
Consistency means jack if your doing it wrong.

The question is: what's the basics of a good training program that achieves the following goals:
So assuming its not asking for the break down of the program, just the basic fundamentals of of one.
